Short Nose Deformity
Short nose deformity is the nose feature with lifted nose tip, unturned nostrils and can be seen the nostrils more than usual that similar to the Short nose deformity. In the medical term called "Contracted nose deformity" or "Upturn nose". The Short nose deformity problem that happens after the rhinoplasty is very difficult to solve and the surgeon has to truly understand the deformity of the internal structure of the nose.
The cause of Short nose deformity after the rhinoplasty is due to the shrinking of the nose s tissues which caused by
1. Capsular contracture
In the present, the popular rhinoplasty technique in Thailand is by implanting single silicone from the nose bridge to the nose tip. it might be various shapes of prefabricated silicone or a piece of silicone that will be carving into the desired shape. when the silicone is implanted in the body, body will create scar tissue surrounding the silicone called "capsule". In some patients the capsule may contracted which if it s happen along vertical axis o the the nose, it will make the nasal bridge shorter and resulting in short nose deformity.
2.Scar contracture
Usually found in the patient who ever had the inflammaton and infection after rhinoplasty, multiple rhinoplasty, having the foreign body inside the nose such as the injection of liquid silicone, the injection of the artificial filler or the others foreign body, the contracted scar will cause the same issue as the capsular contracture.
The contraction of nose tissues also affect the internal structure of the nose by dragging the cartilage in the nose tip area upward to the improper position.
If this Short nose deformity is not corrected, the bad result that can clearly be seen is the aesthetic. The nose tip will be lifted to expose the nostrils, the nose tip skin will thin and may cause the extrusion of silicone and the significant issue that has always been overlooked is the respiration, the overlifted nose tip has effect to the excessive and the speed of the air breathed through the sinus which is too fast for the sinus to adjust the moisture of the air in time and will cause the dryness of sinus and followed by Sinusitis.
The best method to solve this Short nose deformity is by using open Rhinoplasty technique. By this technique can remove the capsular or the whole internal scars under direct vision which allows the contracted skin can be expanded and change the positions of internal structure that are out of place and rearrange the internal structure which make longer shape of nose tip without using the silicone. This will reduce the chance of repeatly having the Short nose deformity issue that is the advantage above the close Rhinoplasty technique.
Effort to correct by using close Rhinoplasty technique may not be able to skirt all of the capsule or the internal scars which can t allow the skin to fully expand and the malposition internal structure can t get any rearrangement. Using longer silicone to fix the short nose tip, even by also using ear cartilage or tissues to patch the silicone, in the first period the nose may turn out beautifully but after a while the skin that can t fully expand will pull back and cause the tilted or extrusion of silicone.
